Will statin medications permanently harm my liver?
Statins are one of the most researched medications of all time. Statins are used to effectively lower cholesterol and, more importantly, reduce the risk of heart attacks and strokes. Many people worry that statins might harm the liver. This is based on data from early statin trials that revealed an increase in liver enzymes in some patients on statin medications. However, over the decades, high-quality studies have shown that this fear is largely unfounded. In fact, many patients can take statins safely, even patients with established liver disease (like fatty liver) or mildly abnormal liver enzymes at baseline.
How common is liver injury from statins?
Uncommon. Drug-induced liver injury (DILI) is a rare but serious reaction to medications. Between 2004 and 2012, only 22 out of 1188 cases of liver injury reported in a national U.S. network were linked to statin use – less than 2% of all cases (Russo et al., 2014). Another study from Sweden found that only 1.2 out of every 100,000 people> who took statin had a reported case of liver injury (Bjornsson et al, 2012). These numbers show that liver injury from statins is extremely rare, roughly .001%.
What about mild increases in liver tests?
Mild increases in liver enzymes (like ALT or AST) can happen with many medications, including statins. But these changes don’t necessarily mean liver damage. In fact, only 1-3% of people taking statins develop mild liver enzyme elevations – and these often go away on their own without stopping the medication (Russo et al. 2014).
In a large meta-analysis of five randomized controlled trials, patients who already had slightly elevated liver enzymes before starting statins (1-3 times the normal range) were followed over time. Those who took statins had fewer heart attacks and strokes than those who didn’ – and importantly, their liver tests did not get worse even on higher doses of statins (Masson et al, 2021).
Additional high-quality research shows no liver harm from statins. A 2022 review by the U.S. Preventive Services Task Force, which analyzed multiple randomized controlled trials, found that moderate-intensity statins lower the risk of heart disease without causing significant side effects – including no significant increase in the liver enzyme, ALT (Chou et al, 2022).
What should I know about statins if I have fatty liver disease?
Metabolic dysfunction-associated steatotic liver disease, or MASLD (previously called "fatty liver disease"), is the most common liver condition in the world. It affects roughly 30% of all adults and is even more common in people with diabetes, obesity, or other metabolic conditions (Younossi et al 2025).
Having a fatty liver is a warning sign that your heart may also be at risk. In fact, cardiovascular disease, not liver failure, is the number one cause of death in people with MASLD (Huber et al 2025; Ochoa-Allemant et al 2025).
Liver and gastroenterology society guidelines now recommend that patients with MASLD receive statin therapy when it is indicated for cardiovascular protection. Also, the recently released 2026 AHA/ACC/ADA/ASN Cardiovascular-Kidney-Metabolic (CKM) Guideline states: "Statins are safe and effective in patients with MASLD and should be used according to guidelines" (Ndumele et al 2026).
In short, experts recommend statins to their patients when heart risk is present, because protecting the heart is very important for people with fatty liver disease.
Can statins actually help protect the liver?
Recent data suggests that statin medications might actually benefit certain types of liver disease, including MASLD. Results from meta-analyses and systematic reviews showed that participants with MASLD on statin therapy had significantly lower liver enzyme levels than those not on statin therapy (Dai 2022; Fatima 2021). Some studies have even shown that the amount of liver fat and the degree of scarring (fibrosis) improved with statins (Fatima 2021).
A 2024 study of primary care patients with MASLD found that those prescribed a statin had a 40% lower risk of their liver scarring progressing to a dangerous level over about three years, compared with patients not on a statin. This benefit was seen with both moderate- and high-intensity statins (Schreiner et al 2024). A 2025 retrospective review confirmed that liver enzyme levels in MASLD patients remained stable over 12 months of statin therapy, with no worsening regardless of statin type or dose (Wong et al 2025). A comprehensive 2026 review of the medical literature concluded that statins are not only safe in MASLD but may also reduce liver fat, slow liver scarring, and lower the risk of liver cancer through anti-inflammatory and anti-scarring effects that go beyond cholesterol lowering alone (Commins et al 2026).
While this evidence continues to grow, it is encouraging that the benefit of statins could go beyond treating cardiovascular disease and potentially also help protect the liver in patients with MASLD.
