You’ll be paired with a health professional who’ll work with you through confidential, over-the-phone sessions to give support, encouragement and expert guidance.

Our caring, dedicated health coaches will help you implement and adhere to your personal action plan. Your action plan includes individualized recommendations on exercise training/physical activity, nutrition, weight management, stress management, quality sleep, tobacco cessation and other important preventive topics, as needed.

You’ll begin with the lifestyle changes that are important to you at this time. You’ll define your wellness vision – the health and lifestyle you desire for the future. Your coach will work with you to get where you want to be. Our coaches listen to your needs, preferences and circumstances and help you develop a plan that is individualized for you. Your coach will help you hold yourself accountable for your actions and support you every step of the way to better health for life. A variety of resources, tools and technologies are available to keep you on track for success, including online applications and text messaging options.

The program is designed to:

  • Help you make and adhere to meaningful lifestyle changes – for example, regular exercise, healthy eating, weight management, stress management and tobacco cessation
  • Teach you other important steps you can take to optimize your health – for example, necessary preventive exams and immunizations
  • Educate you about your risk factors for chronic disease – for example, high blood pressure, abnormal cholesterol/triglycerides, diabetes, overweight/obesity and sleep disorders
  • Assist you in complying with certain other aspects of your medical care – for example, taking your medications as prescribed

We know that time is a barrier to leading a healthy lifestyle, so we’ve designed our coaching program to be as time efficient as possible. In fact, many participants find it helpful to address time management early in their program. Except for the first session, which requires approximately 30 minutes, all other sessions are approximately 15 minutes. Over the course of the year, up to 18 telephone appointments are scheduled at times that are convenient for you. Our coaches are trained to use communication techniques that focus on your needs and maximize the time you have together. Of course, to get the most from the program, you’ll need to spend some time reviewing materials prior to and after your coaching sessions. Mostly, you’ll be putting what you learn into action in your daily life, which should help you achieve a sense of balance and feel in control of your life.

You will work with your health coach to implement your personal action plan and achieve your individualized short- and long-term goals. It will be your responsibility to keep your coaching appointments, try your best to put the plan into action and stay on track. If you get off track, your coach will help you learn from the experience and start moving forward again. Change is never easy, but it is possible. Our program has worked for thousands of participants just like you.

Our lifestyle health coaches are health professionals who come from various disciplines including nutrition and dietetics, exercise science, public health and health education, nursing and health psychology. All coaches have a four-year college degree in a health-related field as a minimum requirement and many have advanced degrees. Each is an experienced coach who truly cares about the welfare of our participants. They are great listeners and communicators and have received special training in behavior change processes and techniques. You’ll love working with our dedicated health coaches.

Your physician may have recommended this program to you because he or she knows it works. If implemented with commitment, it will help you make improvements in your lifestyle that will benefit your health. Your physician is familiar with the content of the program and knows the science that is the foundation of the program. We will inform your physician when you enroll in the program and he or she will have an opportunity to stay involved in the following ways as long as you participate:

  • Provide the current biometric measurements for your initial assessment
  • Review your baseline health assessment report, goals and action plan and future progress reports and make changes, if needed
  • Provide clearance for you to exercise, if needed
  • Review your medications and make changes, if needed
  • Order any additional tests that may be indicated
  • Know the number of appointments you have completed and the topics you have covered with your coach
  • Review your follow-up reports and revised goals/action plan and make changes, if needed

Of course, your physician remains in control of your treatment and you should always follow your physician’s advice. Your lifestyle health coach may refer you to your physician during the course of the program. If you are referred to your physician, you should make an appointment as soon as possible and report the results of your visit to your health coach.

No, probably not. You are probably used to your health plan covering a portion of your health care costs. But, you probably have a co-payment. The cost of our individual sessions is about the same as a co-payment for a doctor visit or prescription medication on many health plans. And, you save time and effort because you don’t have to travel to the doctor’s office or another location for an appointment. Also, think about cost vs. value. The average daily cost of the program is about the same as the cost of a can of soda or cup of coffee. When asked about the value of good health – most people say “priceless!” What do you say? These versions of our lifestyle management program are designed especially for people who are ready to make lifestyle changes and have the commitment, discipline and support to manage their own program. Programs are available to help you make and adhere to lifestyle changes in each of these specific key areas: physical activity/exercise, nutrition, weight management, stress management and tobacco cessation.

An advantage of a self-help program is that you can work at your own pace. You will participate via the Internet, so you will have the convenience of completing assessments and accessing and printing materials at times that work for you.

Our most popular and successful lifestyle management program is delivered by specially trained health professionals, called health coaches. Because our self-help programs use the same high quality educational materials as our coaching program, you will see references to health coaches throughout your reports and materials. While these references won’t apply to you, they emphasize the benefit of having the expertise of the health coach to guide and support you. Our self-help programs use the same evidence-based goals and action plans, educational kits, audios, diaries, follow-up evaluations and progress reports as our coach-assisted program. The self-help programs focus on a specific topic (for example, nutrition, exercise, weight management, stress management, tobacco cessation) while the coached program is comprehensive (all of the lifestyle management topics are addressed in an integrated manner). Participants in a self-help program work at their own pace, but typically spend about 12 weeks on a specific program. For the coach-assisted program, telephone appointments are typically scheduled over a nine to 12 month period of time.

You may work at your own pace, but typically participants spend about 12 weeks to complete a self-help program. We suggest that you try to cover one or two topics each week.

No. You may enroll in only one self-help program at a time. However, you may participate in additional programs during the course of the year.

Many of our participants in self-help programs decide to enroll in a coach-assisted program. It’s very easy to make the change at any time.
