Manage My Condition

If you have been diagnosed with heart disease, you should be working with your physician to evaluate your treatment and long-term health management options.  Our experience shows that patients that are more actively involved in managing their heart disease have better outcomes than those that take a passive approach.   As a heart patient, you should work closely with your cardiologist to understand what you can do to maximize your chances for a long and healthy life.  This includes taking medication as prescribed, leading an active lifestyle, reducing stress, eating healthfully, and living a healthy lifestyle.

In determining the treatment approach that is right for you, we encourage you to work with your physician to understand how the guidelines established by the medical community for managing your condition can be best applied to your situation.  To assist you, the most important points of the guidelines for the most common heart disease conditions are provided in this section.

In the near future, we will also be adding online tools to this section, which will help you to track important data, such as weight, blood pressure and physical activity, to help you and your physician better manage your health.
