Description

The angiotensin receptor blockers are very similar to ACE-inhibitors in that they block the harmful effects of the kidney on the heart and blood vessels and are among first line treatments for high blood pressure. In addition, they have a strong role in helping to make the heart stronger for patients with congestive or chronic heart failure and are a cornerstone of medical therapy in that area. There is also data that ACEIs and/or ARBs help to prevent adverse cardiovascular events (such as heart attacks) for patients at high risk, although this is somewhat controversial. They also are widely used to prevent kidney function from worsening in patients with diabetes or preexisting kidney disease. For the most part, they are considered therapeutically similar to ACE-inhibitors, but tend to have slightly fewer side effects.

When might my doctor prescribe this medication?

  • High blood pressure (or hypertension)
  • Congestive or chronic heart failure (CHF)
  • Chronic kidney failure
  • Diabetes to prevent or lessen the amount of kidney failure
  • After a heart attack or if I have been diagnosed with coronary artery disease (CAD)

I should NOT take this medication if:

  • I have had severe allergic reactions to losartan or other ARBs in the past
  • I have had a specific reaction to ARBs in the past where there was swelling around my face and/or lips and throat (called angioedema)
  • Strong caution should be used if I have had this reaction to ACE-inhibitors as there is some potential for the same reaction to happen with ARBs

Adverse Effects (common)

  • Low blood pressure
  • Fatigue

Adverse Effects (rare, but serious)

  • Swelling around the mouth, lips and throat (angioedema)
  • High blood levels of potassium
  • Acute kidney failure

Common Adult Doses and Forms

Losartan (Cozaar): 25-50 mg once daily up to a maximum of 100 mg once daily (can also be given twice daily)

Dosage Forms:
Losartan (Cozaar) (tablet): 25 mg, 50 mg, 100 mg

How should I take this medication?

Losartan can be taken with or without food. Do not use if pregnant or plan on becoming pregnant. Report any swelling of the face, lips, tongue, throat, or unusual difficulty breathing immediately to your doctor.
